INTEGRATED TEAM CARE
Our Integrated Team Care program aims to improve Aboriginal and Torres Strait Islander access to primary health care services within the Great Southern, Coastal & Southern Wheatbelt regions.
We support Aboriginal and Torres Strait Islander clients with chronic disease to link them in with the support they need to better manage their health
If you would like to join the program, our team can talk to you and help you to take a health check. If your GP has written a care plan for you, we can then book the appointments you need and support you in other ways to better manage your health. As a team, our staff will help you to feel more confident about your health.

MENTAL HEALTH WELLBEING & RESILIENCY PROGRAM
This community engagement program is designed to support the physical and psychological health of Aboriginal communities in the Great Southern and the Wheatbelt. Through group and individual support programs we hope to encourage social health and wellbeing, while nurturing spirituality and cultural integrity.
The aims of the program include increasing assisted referrals to appropriate clinical and community supports; raising awareness of factors affecting mental illness, drug and alcohol abuse and suicide; increasing access to culturally appropriate services.
Programs are currently operating in Moora, Narrogin, Merredin, Gnowangerup, Mt Barker and Tambellup.

STRENGTHENING ABORIGINAL HEALTH PROGRAM
Through the Strengthening Aboriginal Health Program, Amity Health is able to partner with general practice and Great Southern Aboriginal Health to provide clinics for clients to have their health assessed and regularly checked.
This project aims to increase the number of Aboriginal clients who receive chronic disease screening services, care plans, care plan reviews or health checks through Medicare.
Clinics are provided in Albany, Mt Barker, Katanning, Kojonup, Tambellup and Gnowangerup.
